Choose a real failure with a clear boundary
A failure story needs an intended outcome that you did not achieve. Working too hard or caring too much is not a useful substitute. Choose a situation where you can explain your responsibility without exposing confidential information or inventing a harmless mistake.
Define what failed: a deadline, a prediction, a customer outcome or a process. Separate the result from your identity. You are explaining a decision and what you learned, not arguing that you are either perfect or incapable.
Explain the assumption behind the mistake
The most useful part often comes before the failure. What did you assume, what evidence did you miss and why did the approach seem reasonable at the time? This gives the interviewer something concrete to discuss.
For example, an estimate based only on implementation time can miss review, data migration and testing. Say which parts you overlooked and when the gap became visible. Do not hide your decision behind a vague statement that the team was busy.
Worked answer: a missed delivery estimate
Illustrative answer: 'I committed to a reporting change after estimating the code work, but I did not include validation with the finance team. Two days before delivery, we found different definitions for a key metric and missed the agreed date. I told the project owner, split the validated report from the disputed metric and arranged a definition review. We delivered the narrower report first. I now include acceptance criteria and reviewer availability before committing to a date.'
Use your actual timeline and outcome. The answer does not claim that recovery erased the missed commitment.
Show the recovery without rewriting the result
State who was affected, how you communicated and what you did to limit further damage. Then explain what remained unresolved. A failure can lead to a useful improvement while still being a failure.
Avoid adding a sudden triumphant ending just to make the story sound impressive. If the project was canceled, explain how you preserved useful learning or supported the transition. If your correction did not work immediately, describe the next adjustment.
Demonstrate a changed habit
A credible lesson has a later example. If you now review assumptions earlier, describe a subsequent project where that prevented a similar problem. If you have not had the chance to apply the lesson, say what process you have put in place and how you plan to evaluate it.
Prepare the question 'What would you do differently with the information you had then?' This is more revealing than hindsight alone. It tests whether your new method was actually available at the time.
Practice exercise
Write five bullets: intended result, your mistaken assumption, observed failure, recovery action and later behavior. Rehearse without the words 'basically' or 'it was not my fault.' Ask a partner to interrupt after the second bullet and challenge your reasoning before you reveal the outcome.
